First Visit

At the Chicago Center for Cosmetic and Implant Dentistry, we understand there may be many reasons that you may be seeking treatment. Your initial appointment will be focused on identifying your goals of treatment, whether for health, cosmetics, function or any combination of these. During the appointment a thorough head and neck and oral examination will be completed. From this, we will develop together a treatment plan and it will be presented to you outlining the sequence of treatment to assist you in reaching your dental goals.

In an effort to use your time most effectively, please assist us by providing the following information (if available) at the time of your consultation.

1. A referral slip from your dentist indicating the nature of the referral.
2. X-Rays. If your dentist or physician has taken x-rays, you may request that they forward them to our office. If time will not allow, please pick them up and bring them with you. If additional x-rays are necessary, they can be taken at our facility.
3. A list of medications you are presently taking. Please alert our office if you have a medical condition that may require antibiotic medication prior to the examination or treatment (ie heart murmur, artificial heart valves or joints, history of rheumatic fever, etc.).
